All students registering with QSAS must complete the student intake form located in Ventus and submit documentation that meets the documentation criteria. We do not recommend the submission of private and confidential medical documentation by email as it is not a secure means of transmission.
Students must provide documentation that:
- Confirms that they are a student with a disability (disclosure of diagnosis not required).
- Contains information about functional limitations within the academic environment.
- Indicates the duration and severity of the disability or impairment.
Students who submit documentation that meets documentation criteria may °ù±ð³¦±ð¾±±¹±ð permanent a³¦³¦´Ç³¾³¾´Ç»å²¹³Ù¾±´Ç²Ô²õ.&²Ô²ú²õ±è; Students who are unable to submit documentation that meets QSAS documentation criteria, but who are able to submit some documentation, may °ù±ð³¦±ð¾±±¹±ð interim a³¦³¦´Ç³¾³¾´Ç»å²¹³Ù¾±´Ç²Ô²õ.&²Ô²ú²õ±è;
Accessibility Advisors may request additional documentation. We are only able to accept documentation in English. It is the students' responsibility to have documentation from other languages translated into English before submitting.

- Attention-Deficit Hyperactivity Disorder (ADHD), Attention Deficit Disorder (ADD)
- Neuro-diverse perspectives including, but not limited to, Autism Spectrum Disorder (ASD), Tourette Syndrome, etc.

Examples include:
- Hearing loss related to, but not limited to, Deaf, deafened, hard of hearing (may be used to rule out hearing issues for individuals experiencing Central Auditory Processing Disorder)
- Vision loss related to, but not limited to, macular degeneration; cataracts; glaucoma; diabetic retinopathy; blindness

- Brain injuries including, but not limited to, concussion, acquired brain injury (ABI), etc.
