The Department of Environmental Health & Safety is releasing a newly created Hazard Reporting Standard Operating Procedure (SOP) along with an updated Incident Reporting & Investigation SOP. Reporting hazards and incidents provides an opportunity to remove or reduce a hazard to avoid injuries in the future, as well as collects the information that is required for legislated reporting.
The Hazard Reporting SOP (SOP-Safety-23), introduces a university wide and consistent process for employees to report the hazards they see or experience. The ability to report hazards is the first step towards making the environment safer and reinforces the Internal Responsibility System. The SOP outlines a process by which employees can report these hazards to supervisors and a means of documenting and tracking follow-up on the new report form. The Incident Reporting & Investigation SOP (SOP-Incidents/WSIB-02), features an updated reporting process and introduces a single reporting form to streamline the incident reporting process. This SOP reminds people of the importance or reporting all incidents to enable Queen’s to meet its legislative reporting requirements and provides supervisors with guidance on investigating the cause of incidents.
