Abstract:
Some of the largest issues facing Ontario鈥檚 healthcare system are the transitions between care settings and the gaps in health care delivery. The Patient Ombudsman鈥檚 office has been travelling around the province to learn more about the patient and caregiver journey, the challenges health sector organizations face and how different experiences can be depending on where people live. Ms. Elliott will share her observations, regional learnings and perspectives derived from numerous patient consultations and outreach activities.
